What documents do you have bar owners / private party venue holders fill out when you do a gig for them? Do you just ask for a signature, date, and dollar amount?

Don't generally use a contract with bars. Most don't like to sign them & there are ways they can legally get out of them as well.
If they don't pay or cancel a night, I move on to another club.

Is this pre or post gig? Record keeping is up to you. Keep a solid up to date ledger book of all of your transactions.
Is that what you are asking?

I keep contracts with all booking parties and clubs. as well as a leger for my accountant if all income and expenses for both the studio and audio company as well as personal.
it makes life so much easier come tax time.
